Housing / Shelter and Mental Health - Adult

These settings enable individuals to develop and/or maintain their social and life skills to work towards the highest level of independence possible.
If individual is currently receiving mental health services or have been referred to a community mental health and substance use team, they may be eligible for mental health supported housing. Not all persons receiving services through the community mental health and substance use team require supported housing.
Discuss housing needs with the community mental health clinician. If individuals do not have a clinician but think they are in need of mental health supported housing support, contact the community mental health and substance use centre, click here.
First of all, discuss with TH family member or friend to see if they are interested in mental health supported housing. Participation is voluntary. If the loved one who has mental health challenges is interested, support them by contacting their community mental health and substance use centre, click here.
Offer a variety of bed based supports, from licensed and assisted living programs, to providing services to clients in supported independent living arrangements in their community. These services may be provided by contracted service agencies. Click here to find out
This Program is for:
- Adults 19+ years of age who are diagnosed with a serious and persistent mental illness.
- Eligible individuals who are receiving services from their community mental health and substance use team.
To Access Mental Health Supported Housing and Residential Program Services:
- Some of the mental health supported housing programs require a referral that is completed by health care providers (e.g. family doctor, nurse practitioner) in consultation with the individual’s community mental health and substance use centre.
- Other mental health supported housing programs require an application that is completed by the individual seeking housing with support of their mental health professional.
- Eligibility is based on an individual’s needs as identified by their community mental health and substance use team.
