These facilities provide long term care for adults who can no longer live safely or independently at home due to their need for 24-hour nursing supervision in a secure environment, and their unpredictable or complex care needs which can no longer be managed in their home setting. Additional services include personal care assistance, end-of-life care, meals, nutritional support, social work, and recreational activities.
Do not contact the care home directly to reserve a space as access to long term care is made through assigned case managers who first determine eligibility.
*Some long term care homes are operated privately by an outside party.
Eligibility
Clients must meet Home and Community Care requirements and criteria for admission. A health care needs assessment will be completed to confirm eligibility. See the eligibility requirements for home and community care section of the website.
Referral
Referral by case manager at a Home and Community Care office is required. New clients can contact the Fraser Health Access Line: Home and Community Care (previously called the Home Health Service Line) by calling 1-855-412-2121 (open 7:00 a.m. – 9:00 p.m., 365 days a year) or they can book a telephone appointment by filling out their online form. Current clients can contact their Home Health location. Current clients can contact their Home Health location. The patient can call, or someone can call on their behalf: family member, hospital, family doctor, friend, neighbour, community group or government agency. The patient will be assessed by a case manager to evaluate their needs etc. If appropriate, a care plan will be developed.
Fees
There is a monthly accommodation fee; additional fees may apply for incidental services.
