Offers education and exercise classes to help individuals better manage their chronic condition.
The iConnect Health Centre provides various education and exercise classes for patients to be better able to manage their health conditions. The Health Centre offers classes focused on Cardiac Rehabilitation and as well as those offered by the Diabetes Health Centre.

For diabetes education classes a referral from a physician is required. Interaction education sessions are currently being held on Zoom by different health care professionals. These sessions span 4 weeks, and upon completion the individual has an intake with the cardiac rehabilitation team to have an individualized exercise and care plan developed. .

For patients that do not have access to Zoom, the Cardiac Rehab team has a home-based program. You will have an intake assessment with a nurse, and then an exercise specialist will go over a home exercise program and follow up via phone calls. The team usually starts off with more frequent calls, and then as needed for the 16 week duration. Language barriers can be met as there are options for delivery in different languages.

Diabetes Education Centre at the iConnect Health Centre in White Rock has gone virtual for all appointments due to COVID.
