Physician referral required for education classes. Physician approval required for exercise portion. Exercise treadmill test conducted at patient intake appointment. In the exercise portion of the rehab program, clients will be monitored within the high risk classes, offering in person, virtual and remote monitoring exercise monitoring and support.
The program also provides comprehensive Cardiac Rehabilitation Education Self-Management Support Classes focusing on lifestyle management topics that promote heart health after a cardiac event. Interdisciplinary team of health professionals available each week to educate and assist clients towards healthy lifestyle changes. Virtual education sessions can be accessed immediately post discharge with referral. Call clinic for class times and schedules. Education classes run for 6 consecutive weeks and exercise classes run for 12 consecutive weeks, once per week.
Fees may apply for optional workshops such as smoking cessation, lifestyle management, community-based exercise therapy or stress management.
