Seniors' Community Connector - Social Prescribing Program - Maple Ridge/Pitt Meadows
Caregiver Support, Food, Seniors Services, and Social / Recreational
Provided by Maple Ridge/Pitt Meadows Community Services
When a Healthcare provider identifies a patient who could benefit from the program, they can send a 'prescription' to the Seniors' Community Connector who will support the patient in accessing suitable local sources of support.
The following types of older adults living in Maple Ridge or Pitt Meadows can be referred to the program:
- social isolation;
- emotional problems;
- major life events such as loss of a spouse;
- chronic diseases;
- physical inactivity;
- poor nutrition and/or food security concerns;
- poor health outcomes associated with social determinants of health
- frequent use of primary health care.
