On Track Program - A Cardiovascular Health Program

A health education and supervised exercise program for people who would benefit from clinical support in making lifestyle changes.
The goal of the 'On Track' program is to provide evidence-based support to reduce cardiovascular risk and improve mental health and overall well-being. This program is for patients with established risk factors for cardiovascular disease such as Diabetes, Hypertension, Dyslipidemia, Metabolic Syndrome as well as mild to moderate depression and/or anxiety. Exclusion criteria are for people with a diagnosis of cardiovascular disease. The program is run in partnership with Interior Health.
Participants will gain skills in mindful eating, label reading, stress management, goal setting and action planning and cognitive behaviour techniques such as thinking realistically and problem solving.
Format: Sessions run weekly for 10–11 weeks with guidance from Interior Health and City of Kamloops staff. The program runs in the Fall (September - December), Winter (January - March), and Spring (April to June).
To qualify for the program, participants need to get a referral from their physician and go through a screening process (which is done by the Program Coordinator). Participants pay a $35 fee for the program.
Who is On Track for:
- Individuals with pre-diabetes, diabetes, hypertension, dyslipidemia, metabolic syndrome, inactivity, and mild to moderate depression and/or anxiety.
- Not for people with established cardiovascular disease (please refer to VIP - Cardiac Rehab program).
If participants cannot afford the program fee, they can receive a subsidy (of up to 80% of the fee) by applying to the Affordable Recreation for Community Health (ARCH) - Financial Assistance for Recreation in Kamloops program.
