Assessment for eligibility for Home Support services, palliative care, wound care, long term care and assisted living.
What services do community health nurses provide?
  • Palliative care
  • Wound care services: If a person gets wound care through Fraser Health's home health services, they will receive two weeks of supplies from Fraser Health, but they are responsible for the rest.
  • Intravenous (IV) medications: If a person is getting IV medications through Fraser Health's services, Fraser Health will provide the medication and supplies needed until the treatment is finished. If it's not an approved medication in BC or experimental, it won't be covered.
  • Medication management education and support based on needs assessment.
  • Health education on chronic diseases, healthy living, family and friend-provided care.
  • Supports to help you be more independent (i.e. financial support, housing services, medical care, etc.).
  • Assessment for need of caregiver support services.
  • Care management and navigation, including coordination and referral to professional health services and community supports.

    Home Health Case Management
Case managers coordinate care for people with complex health needs requiring extra, ongoing support to live at home independently.
Their goal is to help people live at home safely and avoid unnecessary or premature admission to a hospital or care facility.
Case management is one of the many Home Health services offered by Fraser Health.

What types of programs and services are offered?
A case manager works closely with clients, their family and friends to:
  • Assess clients' needs
  • Determine clients' eligibility for services
  • Determine the costs for certain services, based on clients' financial situation
  • Plan, coordinate and monitor multiple care services
  • Arrange relief for caregivers
  • Assess whether clients need to move to an assisted living or long term care facility

    Some services are provided free of charge while others include a cost based on your after tax income. Contact your local Home Health location to inquire about fees.
