Outpatient Rehabilitation Clinics

Focus on enabling people to self-manage their rehabilitation goals, improve their ability to participate in daily activities and improve their quality of life after discharge from the hospital or inpatient rehabilitation unit.
How To Access This Service:

Individual must meet all of the following criteria:
  • Have been referred by hospital, inpatient rehabilitation care team or health care provider.
  • Live in any community within the Fraser Health region.
  • Have had a recent medical event; for example, brain injury, stroke or spinal cord injury.
  • Based on individuals recovery, the hospital care team believes that outpatient rehabilitation can improve individuals ability to remain as independent as possible.
  • Want help learning how to do everyday activities independently, such as eating, dressing, communicating and walking.
  • Are willing and able to take part in therapy.

How To Get Referred:

The hospital or inpatient rehabilitation care team will assess individual. If individual qualifies for outpatient rehabilitation, they will discuss it with individual and ask for individuals consent before making a referral. A regional team will screen the referral and one of the team members will let the individual know about the referral decision.

Also accept referrals made by health care providers in the community, for people with functional rehabilitation goals to be more independent (such as dressing and mobility) or for swallowing diagnostic procedures such as modified barium swallow. A health care provider must complete the referral form, click here and include all relevant medical consults and investigative reports.).

Rheumatologists can download the referral form for the Fraser Health Inflammatory Arthritis Program, click here to refer patients for this specialized service.

The referrer is responsible for informing patients of the referral decision and admission information.
