Seniors Outreach is available to support family physicians and community health care providers when a senior needs further assessment and treatment, or to investigate abuse.
Seniors Outreach is an intake service available to support family physicians and community health care providers when a senior needs further assessment and treatment. The intake liaises and provides navigation support based on what the patient's needs are, and can advise what services, assessments, and treatments would be beneficial in areas such as mental health and substance abuse, age-related conditions and other concerns. Seniors Outreach Teams do not do hands-on assessments or treatments, but may be responsible for investigations of abuse and neglect.

Victoria Community Access Centre for South Island - all communities south of Mill Bay, including Greater Victoria, the Southern Gulf Islands and west to Port Renfrew.

Nanaimo Community Access Centre for Central Island - all communities from Mill Bay to Deep Bay, including Gabriola Island, and from Parksville on the east coast to beyond Tofino/Ucluelet on the west coast.

North Island Central Intake for all communities north of Deep Bay, including the Mt. Waddington Regional District and the mainland area adjacent to it.

How To Access This Service
Step 1: Contact your regional community access centre (see below)
Step 2: A Community Access Intake Clinician will conduct an Initial Clinical Assessment
Step 3: A health professional will work with you to develop a care plan to meet your needs.
