Chilliwack & Fraser Health Rural Primary Care Network (PCN) - Traditional Wellness Mentor Services

Provided by Fraser Health

Access wrap-around, team-based, and culturally sensitive primary care and allied services that is interdisciplinary in its approach with our multidisciplinary team.
The PCN provides care at the Chilliwack and Fraser Heath Rural Primary Care Centre (PCC) where patients can access guidance and mentorship that supports their health and wellness while promoting cultural safety and humility in the Sacred Space room through the following:
  • Traditional plants and remedies
  • Holistic teachings
  • Traditional learnings and protocols
  • Preparing and planning for traditional plants and remedies
  • Healing and sharing circles in the Sacred Space room
  • Individual wellness plans
  • Ceremonial support
  • Indigenous languages

Referral criteria for all Chilliwack & Fraser Health Rural PCN services:
  • Patients must be Indigenous, Métis or Inuit, 18 years of age or older.
  • Referral options include: Referrals accepted from primary care providers (i.e., family physicians, nurse practitioners) serving residents in Chilliwack, Agassiz-Harrison, Hope, and the Fraser Canyon areas. Self-referral from Indigenous, Métis or Inuit patients who reside in the Chilliwack or Fraser Health Rural (Hope, Agassiz, Harrison, Fraser Canyon) area. Drop-in options are available on Tuesdays & Thursdays 1:00-4:00 pm (last drop-in time 4:00 pm).
