Seamless Perinatal Health Care Program

Provides support and care to vulnerable pregnant women.
The seamless health care team provides referred women with extra support during their pregnancy. This could be for medical and/or social reasons.

The team will review a women's health care needs, and offer support and information based on the concerns from the referring healthcare provider. Support and information may be in the following areas:
  • Physical Health
  • Mood
  • Coping
  • Community Resources

This program is recommended for:
  • Single parents
  • Parents with limited education, social support and/or financial resources
  • Women who have or are experiencing mental health concerns or relationship safety concerns (e.g. intimate partner violence).
  • A women receiving late prenatal care
  • A women who is medically high risk
  • A women who has a newborn with medical conditions or congenital anomalies

Seamless Perinatal is not an isolated program; it is a component of the Best Beginnings Perinatal Services offered by Fraser Health. Seamless Perinatal is a process for communication, collaboration and coordination of services from community to acute care and from hospital to community for vulnerable women/families. When a vulnerable pregnant women is identified through pre-natal registration process, she is referred to appropriate service provider by the Public Health Nurse in community and then liaised with hospital service providers upon admission for delivery. The client receives appropriate care at the hospital and is followed up in the community. The referral to Seamless can be made by a care provider i.e Physician, Public Health Nurse, Social Worker, PCC or any other care provider.
